Requirements to be a godparent: 1. Be a baptized Catholic and above the age of 16 2. Have received the most holy Sacrament of the Eucharist, 3. Have been Confirmed 4. If married, must be married in the Catholic Church in a valid, sacramental marriage. 5. If single, must be living a life of chastity 6. Must be a practicing Catholic: Attending Mass every Sunday and Holy Day of Obligation, leading a life of faith in which the sacraments of Reconciliation and Eucharist are received regularly. 7. Godparents must not be the father or the mother of the child to be baptized
Godparents:
Must meet all the requirements (see above)
There is to be only one male godparent and/or one female godparent (only one Godparent is required)
Are expected to attend a Catholic Baptism Preparation class and provide documentation
For baptisms held at SMdP, the Godparents must provide this Certificate to serve as a Godparent (see below if one or both Godparents are parishioners of SMdP)
There are two sides to be completed--one by the parish, and one by the godparent.
The document must be signed by the appropriate clergy (or their delegate) and marked with the parish seal.
St. Martin de Porres Parish reserves the right to appoint Godparents should a Godparent not be able to produce a letter of Good Standing or not attend a Catholic Baptism Preparation Class.
PLEASE NOTE that the above certificate to serve as a Godparent is for Godparents who are not parishioners of SMdP. If the Godparents are parishioners of SMdP, a separate form is used.
