• Complete the Parent Registration Form.
• At least one parent must be a practicing, active Catholic committed to raising the child in the Catholic faith.
• Both parents are expected to attend the Baptism Preparation Class. If this is not possible, at least one parent must attend the Baptism Preparation Class.
• Parents must be registered, contributing parishioners, or live within the parish boundaries.
• If parents live outside our boundaries, please submit a Pastoral Letter or Permission Form (with parish seal and pastor’s signature) from your home parish. 

• Complete the Godparent Registration Form below.
• Attend a Baptism Preparation Class at any Catholic parish. (Valid for three years)
• Provide a Letter or Form of Good Standing (with parish seal) from your home parish.
• At least one Catholic godparent is required. If two godparents are chosen, they must be one male and one female.
• Godparents must:
  • Be at least 16 years old.
  • Be fully initiated Catholics (Baptism, Confirmation, Eucharist).
  • Lead a life of faith, including being married in the Catholic Church if married, or not cohabiting if single.
• A baptized non-Catholic may serve as a Christian witness alongside a Catholic godparent. The non-Catholic witness cannot be the godparent.

• Child’s state-issued (long form) birth certificate.
• Letters or Form of Good Standing for each godparent.
• Certificate of completed Baptism Preparation (if taken elsewhere).
• Select godparents who meet all requirements (parents cannot be godparents).

• Submit the Parent Registration Form and Godparent Registration Form online (see below).
• Parents and godparents must attend the Baptism Preparation Class (valid for 3 years).
• Classes are held at the SMDP School Building from 11:00 AM to 1:00 PM on assigned dates.
• Registration is required before attending; no walk-ins.
• Registration will close the week the class is offered.

• Once all requirements are completed, a baptism date will be assigned for your child.

• St. Martin de Porres will be your child’s Church of Record for life, recording all future sacraments.
• Sacraments are not celebrated during Lent or other designated liturgical times. (Excluding emergencies such as danger of death). 

Children who are 7 or older are considered to have reached the age of reason and can be registered here: OCIA Adapted for Children and Adolescents
